Within a wreath of 50 stars, the reverse of the medal features the Presidential Seal with a map of the United States in the background. The inscription “INAUGURATED JANUARY 20, 1977” is under the Presidential Seal with an excerpt from President Carter's Inaugural Address: “THE AMERICAN DREAM ENDURES..WE MUST ONCE AGAIN HAVE FAITH IN OUR COUNTRY... -JIMMY CARTER.”
