The obverse of the medal features a portrait of George W. Bush with two stars to represent his second term and the inscription “GEORGE W. BUSH” centered along the right side of the medal.

The reverse features the north view of the White House and an excerpt from the President George W. Bush's speech on advancing freedom and democracy in the world during his June 2007 visit to Prague centered on the top of the medal: “FREEDOM CAN BE RESISTED AND FREEDOM CAN BE DELAYED – BUT FREEDOM CANNOT BE DENIED.” The date “JUNE 5, 2007” is featured beneath and to the right of the quotation. The inscription “INAUGURATED JANUARY 20, 2005” is centered along the lower border of the medal.
